Here's How to Register Electronically:

1. Listed below are the links to the downloadable PDF versions of files needed for registration. Click the links for "2023-24 REGISTRATION FORM" and for "PARENT PERMISSION FORM" to access fillable versions of these files for download.

2. Fill in all the pertinent information on each form and SAVE to your own computer.  When saving forms, please entitle the saved versions as follows:

   • For the Registration Form:

     • St. Francis Families: save the form with the title "SF_FF_Registration_LastName_FirstName" (filling in with your own Last and First Names).

     • Sts. Mary & Joseph Families: save the form with the title "SMJ_FF_Registration_LastName_FirstName" (filling in with your own Last and First Names).

   • For the Permission Form:

     • St. Francis Families: save the form with the title "SF_FF_Permission_LastName_FirstName" (filling in with your own Last and First Names).

     • Sts. Mary & Joseph Families: save the form with the title "SMJ_FF_Permission_LastName_FirstName" (filling in with your own Last and First Names).

       • Please note that this permission form requires a signature; therefore if possible, please print it first, and after signing it, scan the signed copy and save it as described above for emailing.
